Question of the Week

A 45-year-old homeless man is brought to the emergency room by ambulance after being found intoxicated in a nearby park. He is lethargic and uncooperative with questioning and on examination. His pulse is 86/min, respiratory rate is 16/min, blood pressure is 142/82 mm Hg, and temperature is 38°C (100.4°F). Physical examination reveals poor dentition and several punctate areas on the buccal mucosa that express a thick, yellow exudate and can be traced from superficial openings to deeper soft tissue by a single tract. The patient also has a hard, tender, erythematous lump on his mandible. There is no lymphadenopathy. A sample of the exudate is sent for gram stain and culture.

Which of the following is the most likely causative organism?

A: Acinetobacter baumannii
B: Actinomyces israelii
C: Nocardia asteroides
D: Pseudomonas aeruginosa
E: Staphylococcus aureus

Correct answer:
B: Actinomyces israelii

This patient has poor oral hygiene, sinus tracts on the buccal mucosa that express a yellow exudate, and a nodular mandibular lesion, which suggests he has an actinomycosis infection of the buccal tissue. This condition is characterized by multiple oral abscesses and draining sinuses and is caused by the gram-positive organism Actinomyces israelii.

A. israelii is an anaerobic bacillus that forms long, branching filaments that resemble fungi but are much thinner by comparison (see image). This organism is part of the normal flora of the mouth, colon, and vagina and tends to cause infection in patients with dental caries, extractions, or gingivitis/gingival trauma. Men with poor oral hygiene are at highest risk. The characteristic feature of this organism is the yellow aggregates of bacterial products, known as sulfur granules.

Treatment of A. israelii infection is intravenous penicillin for 2–6 weeks, followed by oral therapy with penicillin or amoxicillin for 6–12 months.

As for the incorrect answers:
  • Nocardia asteroides causes pulmonary infections and brain abscesses in immunocompromised patients.
  • Pseudomonas aeruginosa can manifest as pneumonia in patients with cystic fibrosis or as a secondary infection in burn patients. It forms a blue-green pigment.
  • Acinetobacter baumannii is also an important cause of nosocomial infections, including pneumonia and UTI. It is often highly resistant to antibiotic treatment.
  • Staphylococcus aureus causes skin and soft-tissue infections as well as nosocomial infections. It is the most common cause of skin and soft tissue infections.
